New Patient Registration
We have an open list and welcome requests for registration from patients living in or moving to the practice area.
New patients must live within our practice boundary to be able to register with us - we do not accept patients who live outside this area. If in doubt, please check with Reception.
Please note due to the high levels of patients wishing to register with us, it is taking up to 3 weeks for patients to be registered onto our system. Please ensure you have at least 6 weeks worth of medication from your previous gp practice.
Live within our Boundary Area?
Every GP practice has a specific registration area and our area map is displayed below. We will happily register any patient who lives in this area, please note you may be asked for proof of address. If you move, but remain in our practice area we can continue to be your GP, please let reception know of your new contact details.
Please note: if you move outside of the practice boundary you will be asked to register with another surgery that covers your new address.
Change of Personal Details
If you change your name, address or telephone number please remember to inform the surgery. This may be done either by visiting the surgery, phoning us, submitting our change of personal details form or by updating your record online (you need to be a registered SystmOnline user).
Please remember if you are being seen at the hospital to notify them of any changes.
Temporary Services
An appointment for immediately necessary treatment is only available to people who are not registered at any place in our practice area and who require GP services. People who have incurred an accident or medical emergency should attend the Crawley Urgent Treatment Centre or A&E. If the resulting condition is potentially life threatening an ambulance should be called using the 999 service.
In addition, if you are away from your normal place of residence and are staying within our practice area for more than 24 hours and less than 3 months, you may also request an appointment.
